Rhinoplasty is a surgical procedure that includes a series of different cartilage and bone shaping procedures, which can be performed to improve the nose aesthetically and functionally.

Rhinoplasty Operation and Preparation For Surgery 

Rhinoplasty operation is performed under general anesthesia.  Before the rhinoplasty surgery, the hospitals usually perform the blood tests required for general anesthesia, and the coronavirus PCR test is performed the day before.  Before the rhinoplasty operation, both detailed ear nose and throat examination and digital rhinoplasty animation will be performed in Dr Murat Enoz's office.  On the day of the operation, our patients must remain hungry and thirsty 8 hours before the operation time.  Hospitalization is required at least 2 hours before the operation. For this, it will be sufficient to go to the hospital where the appointment is made and tell you that you have an operation that day and that you will be operated by Dr. Murat Enoz.  Since at least one week before the rhinoplasty operation of whole blood thinning medication, aspirin, ginkgo biloba, fish oil, and it is not suitable to use a variety of analgesic effect with a blood diluent.

How Is Nose Aesthetic Surgery Performed and Which Techniques Are Available?

Rhinoplasty surgery is performed under general anesthesia.  During the rhinoplasty operation, there are different techniques for shaping the cartilage and bone, and different techniques for the incisions at the tip of the nose.  If rhinoplasty is performed by making an incision in the area that separates the nostrils, which we call columella, that is, if there is a visible incision, this open technique is called rhinoplasty.  In this technique, the nasal tip skin is lifted upwards and a hidden incision is made just behind the nostrils and the skin is dissected upwards.  Since the surgeon has full knowledge of the anatomy in this technique, it provides the surgeon with the advantage of vision, especially in severely curved noses, noses with caudal septum deviation, noses with thick skin, and noses that have previously undergone various deformities.  Closed technique rhinoplasty is performed with fractions made through the nose.  There is no visible incision at the tip of the nose, but the amount of surgical trauma is not much different from the open technique.  Generally, surgeons can modify these techniques according to their experience and habits.  

I usually use open technique rhinoplasty in my surgeries.  Especially during thick-skinned rhinoplasty surgeries, I can perform additional procedures for thinning the skin by removing subcutaneous adipose tissue with two-level subcutaneous dissection.  In general, I frequently prefer open technique rhinoplasty because I do revision rhinoplasty and complicated rhinoplasty a lot.  Except for open and closed technique rhinoplasty, rhinoplasty operations are modified depending on the techniques used for shaping the nasal bone.  For example, the technique in which the nasal bone is shaped by cutting the nasal bone with an ultrasonic device is called Piezo rhinoplasty or ultrasonic rhinoplasty.  

There is also a micromotor assisted rhinoplasty operation, in which the nasal bone is shaped with the help of a micro motor, that is, with the micro drilling system.  Today, many different bone shaping methods have been defined that generally reduce heat damage and mechanical trauma to the skin, reduce bleeding, and reduce the pain and bruising experienced by patients after surgery.  You can see that there are many different bone shaping techniques on the internet, for example, there are bone cutting, bone rasping and bone shaping techniques with microscopic assistance or using different metal tools.  I usually perform open technique micromotor assisted rhinoplasty operation.  While the average operation time varies between 2 and 3 hours in primary rhinoplasty surgeries, it can sometimes take 5 or 6 hours in revision rhinoplasty surgeries.  The conditions of the hospitals where rhinoplasty operations are performed are as important as the surgical techniques and the materials used.  Is the highest level of hospital, anesthesia and sterilization conditions, quality of materials being used, the use of appropriate and personalized surgical technique, will benefit the process of passing a comfortable post-operative patients.

Viable Tissue Change in Nose - Simulation Doesn't Always Reflect Results! 

Rhinoplasty surgery is the operation of shaping tissues such as the healing changing cartilage, subcutaneous tissue and bone in the nose.  Digital rhinoplasty animations made before the rhinoplasty operation can never fully reflect the real result.  During rhinoplasty, varying amounts of bone, cartilage, subcutaneous tissue and mucosa are injured.  Complete healing of all these tissues may take more than 2 years in patients with thick skin features, while it takes up to one year in patients with thin skin.  In determining the outcome that will emerge, the postoperative care and healing characteristics of the patient are as effective as the surgery performed by the surgeon.  For example, in patients with pre-curved nose features and bone-cartilage asymmetry, it may occur more rapidly than the previous asymmetry due to bone and cartilage memory after surgery.  especially in patients with c nasal deformity, although a completely symmetrical result seems to have been achieved during the operation;  In the late period, asymmetric light reflections may occur in the nose again.  Another important factor is the skin characteristics of the patient.  Although it is possible to obtain a thin and angular nose during surgery in patients with thick and oily skin features;  In the post-operative period, depending on genetic characteristics, regeneration of the subcutaneous tissue and re-thickening of the skin, a longer-than-normal edema period may result in a large, rounded appearance at the tip of the nose. Therefore, thick-skinned rhinoplasty operations are not accepted by some surgeons.  Another important factor that affects the appearance of the patient's nose shape after the surgery is the care of the patient.  A much larger, edematous and rounded nose may occur in the early period, in addition to the expected nose shape, in patients who do not comply with the recommendations for adequate intranasal care, are smokers, and do not follow the recommendations that will accelerate the mucosal healing on a regular basis, in the early period, in the patients who are in contact with the sun in hot saunas and baths.
